Pregnancy Week 20: What to Expect

Baby's development, your body's changes, common symptoms, what to eat, and what to do this week.

Baby's development at week 20

Halfway point. Baby is now measured crown-to-heel instead of crown-to-rump. The 20-week anatomy scan happens this week or next — a detailed ultrasound that checks every major organ system. Baby's gender is visible on ultrasound for most positions. Baby has clear sleep and wake cycles. Baby can hear your voice clearly. Baby has tiny teeth buds and small hair on the scalp. The umbilical cord is long enough now to allow significant movement.

What's happening in your body

Your uterus is now at the belly button level. Blood volume is up about 40-50% from pre-pregnancy. You may notice pregnancy glow or acne (or both). Possible forgetfulness ("pregnancy brain") is real and hormonal. Bump is large; you can no longer hide it. Some clothes that fit a month ago do not now. Heartburn is common.

Common symptoms at week 20

Visible bump. Bigger appetite. Possible "pregnancy glow" or acne. Forgetfulness/brain fog (hormonal). Stronger fetal kicks. Heartburn. Possible leg cramps at night. Possible mild back pain. Possible mild swelling in feet or ankles. Round ligament pain continues.

When to call your provider

Severe headache, vision changes, sudden swelling in face/hands, upper-right abdominal pain — preeclampsia warning, call provider. Decreased fetal movement after established pattern. Heavy bleeding, signs of preterm labor.

How to feel better this week

The 20-week anatomy scan is the major event this week or next. Bring your partner if possible. It is usually 45 minutes to an hour, and you will see baby's heart, brain, spine, kidneys, and stomach checked in detail. Continue everything else — Kegels, walks, sleeping on your side, water, prenatal vitamin.

Nutrition focus for week 20

Continue all key nutrients. Iron and calcium are at peak need. Aim for 27 mg iron and 1000 mg calcium daily. Add omega-3s (DHA particularly). Healthy fats for baby's brain. Diverse plant foods for fiber and micronutrients.

For your partner

The 20-week scan is the big visual moment of pregnancy for most couples. Plan to be there if possible.

Is this normal?

"Pregnancy brain" is partly hormonal, partly sleep-related, and partly a brain rewiring for caretaking — there is fascinating research showing pregnant brains literally remodel themselves. It comes back postpartum, though many parents joke it never fully resolves.
