TL;DR
Twin registries cost roughly 1.6x a singleton registry, not 2x, because many big items are shared. Always 2 of: car seats, sleep sacks, swaddles, bottles. Only need 1 of: crib (yes, really, for the first 4 to 6 months), monitor, sound machine, glider. Twin-specific essentials: double stroller, twin nursing pillow, hands-free pump, dual diaper organizer.
What you need 2 of
Infant car seats. Non-negotiable. Each twin needs their own.
Bassinets or pack-and-plays. Separate sleep spaces are AAP-recommended for safe sleep.
Sleep sacks (4–6 total, 2 per twin minimum).
Swaddles (6 total, 3 per twin).
Bottles (12 total, 6 per twin if formula or combo feeding).
Pacifiers (8 total).
Going-home outfits in newborn AND 0–3 month sizes (twins are often smaller).
Crib sheets (6 total, 3 per crib).
Newborn hats and mittens (2 each).
Nursing pillows. Actually, a single twin nursing pillow (My Brest Friend Twin) replaces 2 single pillows.
What you only need 1 of
Crib. For the first 4 to 6 months, twins can safely share a crib. After that, you'll need a second.
Sound machine. One in a shared room is fine.
Monitor. Get a 2-camera system, but only one base unit.
Glider/rocker. You can only sit in one at a time.
Dresser. Combine clothes.
Diaper pail. One is enough. It'll fill faster.
Baby tub. Bathe one at a time.
Diaper bag. One big one (Skip Hop Forma XL fits twin gear).
Carrier (sometimes 2). Depends on whether both parents will carry simultaneously.
Twin-specific must-haves
Double stroller ($350–$1,200)
The biggest investment. Three styles to consider:
Side-by-side (City Mini Double). Easier turning, doesn't fit through some doorways.
Tandem in-line (UPPAbaby Vista with second seat). Fits doorways, narrower.
Stack stroller. For use after the toddler stage.
Skip the cheap doubles. You'll be using this for 3+ years.
Twin nursing pillow ($60–$120)
My Brest Friend Twin is the standard. Lets you tandem-feed, which doubles your speed. Critical if breastfeeding twins.
Hands-free wearable pump ($300–$500)
If pumping for twins, a hands-free pump is essential. You don't have time to be hands-tied. Insurance often covers Elvie or Willow with a doctor's note for twin pregnancies.
Dual diaper changing organizer
2 babies = 2x diapers. A wide changing organizer with separated zones for each twin saves dozens of "where did I put the wipes" moments.
Color-coded everything
Pacifiers, bottles, sleep sacks, and bibs in 2 distinct colors. It's the only way to tell whose feed/diaper/nap is whose.
